Dear Eventide Nursing Home Residents, Resident Representatives, and Concerned Families,
Guidance from the Centers for Medicare & Medicaid Services (CMS) requires Eventide Nursing Home to provide updated reports of COVID-19 in our nursing home to residents, resident representatives, and their families. CMS allows these reports to be provided to you through our website. We want to explain the information that will be on our website moving forward and how often the information will be updated.
Specifically, CMS requires that we inform residents, their representatives, and families of either
- a single confirmed infection of COVID-19 in the nursing home or
- three or more nursing home residents or staff with new onset of respiratory symptoms occurring within 72 hours.
We cannot provide information that will personally identify a resident, but we will be providing a cumulative amount of information regarding COVID-19. The cumulative data will include a total amount of cases of residents and staff in the nursing home.
We will provide updates to the cumulative data on COVID-19 on our website at least weekly. We will also update the cumulative data of COVID-19 by 5 p.m. the next calendar day following a subsequent confirmed case of COVID-19 on the campus, or whenever three or more residents or staff with new onset of respiratory symptoms occur within 72 hours of each other.
Finally, we want to assure family members that we will continue to communicate all updates about your loved ones to the assigned resident representative we have on file. We will contact them via phone call on a resident’s change in condition, including if they have suspected or confirmed COVID-19.
The revised notification format is as follows:
Facility: Eventide Nursing Home
Cumulative # of Residents COVID+ as of today: 0
Cumulative # of Staff COVID+ as of today: 4
Reason for Notification:
✔ New COVID+ Resident or Staff
✔ Weekly notice
Please know that we are taking all actions we can to prevent the risk of further transmission of COVID-19. We are screening staff before and after their shift, creating isolation areas, wearing PPE, all staff wearing masks when in the facility, on-going education, cleaning and disinfecting frequently, ending group activities and communal dining, limiting visitation, utilizing telehealth, etc.
If you have any general questions, please contact CEO Mindi Baker at 712.263.3114. You may continue to contact the resident’s nurse for specific resident conditions.
Not required by CMS, but we offer the following transparency for Silveridge Assisted Living:
Cumulative # of Tenants COVID+ as of today: 0
Cumulative # of Staff COVID+ as of today: 0
